Job Title:  Care Coordinator 

 

Reports to: CEO 

 

Hours:  Part - time 

 

Salary:  £25,000 - £27,000 per annum FTE 

 

Location:  Hybrid - Working from home, the THC office in Whitstable (Kent) and face-to-face with clients

What are we looking for? 

​

To be successful in this role, you will need to be professional, proactive, organised, flexible and have excellent verbal and written communication skills.

 

Main responsibilities include providing care coordination to Primary Care Networks:

​​

  • Calling patients to book immunisations and vaccinations.

  • Creating written communications to practices and patients.    

  • Providing navigation of care and support across health and care services.

  • Running audits and searches in a clinical system for meeting target-specific goals and objectives.

​

We are looking for someone with the following:

Skills & Competencies:

 

To be considered for the role, you must have:

​

  • A positive and can-do attitude who can meet targets, stay calm under pressure, and promote a positive, safe work environment.

  • Experience working in general practice or primary care networks.

  • Administrative experience.

  • Confident with IT.

​

Frequently asked questions regarding travel and working from home.

​

THC Primary Care provides services across England. 

If you live close to Whitstable and you want to work from the office, you can. If you want to work remotely, you can, BUT the team and our clients would like to meet you and work alongside you.

​

How much travel is required?

Once a month to work in the office in Kent and some face-to-face client delivery.

​

 

🚫 If you solely want to work from home and can't travel to the THC office in Whistable,  this is not the job for you.

🚫 If you are unable to travel to meet our clients, this is not the job for you. 

​

Will my expenses be covered to travel to the office?

No. It's our company policy that staff are responsible for covering their own travel expenses to come to the office.

​

Expenses to work with clients will be covered. 

​
