What to Focus on in Q2 | A Guide for Primary Care Network Leaders

At THC Primary Care, we provide resources for primary care network leaders, and four times a year, we provide a summary of the PCN DES and a suggested action plan for where to focus your attention each quarter.

Now that we have fewer reporting requirements in the current 24/25 DES specification, our quarterly reminders are not prescriptive, as you will all be focused on different things. The purpose of this reminder is to:
