Chancellor John Healey delivered the Autumn Budget 2026 on 28 October. For the NHS, it adds £190 million for winter, but is any of this money coming to primary care?

What does the Autumn Budget 2026 mean for the NHS?

Is there any new NHS funding?

Yes. £190 million. It is a winter top-up for this year, about 0.09% of the health budget. Everything else is a review, a plan, or money moved from existing funds. The table shows where each figure comes from and where it goes.

Item

Extra?

Where the money goes

Why

£190m for winter, 26-27

Yes

The NHS in England, to manage pressures over winter and the rest of this year. The Budget links it to 25 new or expanded urgent care sites already announced for winter, easing pressure on A&E (Budget 2026, paragraphs 5.13 and 6.7).

Scored as new spending in the Treasury scorecard (Budget 2026, Table 6.1 Policy decisions, line 85). About 0.09% of the £212.5bn health day-to-day budget.

£100m robotic surgery

No

Faster take-up of robot-assisted surgery across England, from 70,000 procedures a year now to 500,000 in 2035. Includes a national robotics registry that uses data and AI to track outcomes. The aim is less invasive surgery and fewer gaps by area and gender (Budget 2026, paragraph 5.14).

Comes from the existing £300m NHS Transformation Fund (Budget 2026, paragraph 6.90).

"£37bn more on health this year"

No

Not a set pot of money. It is total health spending this year compared with the last year of the previous government.

From the Chancellor's speech. Already in spending plans.

Lower health budget lines for 27-28 and 28-29

No

Nowhere. It reflects lower employer pension contributions after the 2024 pensions valuation (Budget 2026, Annex B, Table B.1, note 2).

The Budget says departments are "no worse or better off".

1% cut to departmental capital budgets from 27-28

Unclear

Moved to defence. It helps fund the Defence Investment Plan (Budget 2026, paragraph 2.6).

The Budget does not say whether health is exempt.

When will the bigger NHS funding decisions be made?

In 2027. The NHS productivity and AI review reports in early 2027, followed by the 2027 spending review, which sets NHS budgets for the years ahead.

What else does the Budget 2026 do for the NHS?

  • A review of NHS productivity and AI, led by Mark Bailie, reporting in early 2027 (Budget 2026, paragraph 2.17).

  • An AI Challenge Prize worth £15 million. The first round is on the NHS, with a top award of £2.5 million (Budget 2026, paragraph 2.18).

  • A new ringfenced prevention budget from the 2027 spending review (Budget 2026, paragraph 2.21).

  • A National Care Service, paid for by adjusting the Triple Lock from April 2030. The Casey Commission now reports in 2027 (Budget 2026, paragraph 5.24).

What does the Budget mean for NHS employers?

  • Tax-free mileage is now permanent at 55p a mile for the first 10,000 miles and 35p after (Budget 2026, paragraph 6.213).

  • Visa sponsorship fees paid by employers are no longer a taxable benefit, from 28 October 2026 (Budget 2026, paragraph 6.15).

  • VAT on supplying locum doctors (medical deputies) changes from 1 May 2027. No detail yet (Budget 2026, paragraph 6.185).

Is there anything for primary care and general practice?

Primary care is not explicitly mentioned in the Budget. Neither are GPs, general practice, community pharmacy, dentistry, optometry, or the NHS pay award.

What does the Autumn Budget 2026 mean for primary care providers as small businesses?

This covers GP practices, community pharmacy, dentistry and optometry.

Headline rates of income tax, National Insurance and VAT do not change (Budget 2026, paragraph 2.29). The Employment Allowance and VAT registration threshold are not mentioned.
