 Do you need support with booking your patients for their immunisations and vaccinations?

What can we offer?

At THC Primary Care, we have a team of experienced Care Coordinators who are trained on SystmOne and EMIS. They can relieve your practices and Primary Care Network of this process to free you up to focus on other areas.  

How will this help?

By proactively calling patients ahead of the appointment, our service reduces DNAs and saves valuable nursing time.  

 

Our team will work with practices and PCNs to process:  

 

  • Outstanding immunisations 

  • Incomplete immunisations reports 

  • Routine immunisation schedules and recall schedules 

​

This service will also support QOF and IIF indicators. 

What does this service specifically cover?

The service specifically covers,  

 

  • Call and recall for flu vaccinations   

  • Call and recall for Covid-19 vaccinations   

  • Call and recall for Pneumonia vaccinations   

  • Call and recall for meningitis vaccinations 

  • Call and recall for Shingles vaccinations   

  • Routine childhood vaccinations  

  • Monitoring vaccination history requests from the Childhood Health Information Service 

  • Adding Newborn Blood Spot (NBBS) to patient records 

  • Monitoring vaccines administered at school   

  • Referrals to Health visitors  

  • Monitoring DNAs   

  • Call/recall for hypertension follow-up

Our care coordinators can also request blood tests. 

What is the delivery model?

The service is delivered remotely which will involve online meetings with the team to establish the process. 

 

Safety

Each Care Coordinator has completed the required statutory and mandatory training which includes the recently added Oliver Mcgowan training - Autism and LD training. 

 

How is the service funded?

To date, our clients have used the following funding streams when commissioning services from THC:

 

  • Core funding  

  • Development funding  

  • Leadership and development funding  

  • Impact and investment fund  

  • Network participation fund  

  • Care Coordinator ARRS funding  

What impact could we make?

We have reduced the number of children missing their vaccinations by 50% and increased the number of children who have missing immunisation records by 50% in 4 months at Culcheth Medical Centre, Warrington.

 

 

This service also enables networks to utilise PCN funding and secure reimbursement based on achievement against investment and impact targets.
